Located in the heart of Jackson, Tennessee, this essential program utilizes an eight-bed house offering private rooms to homeless men dealing with co-occurring disabilities and mental health challenges who have not found success via traditional programs. We seek to connect these individuals with the resources they desperately need in order to achieve self-sufficiency: case management (including medication management, transportation assistance, and goal setting) and community services provided through referrals to organizations in the West TN Continuum of Care (including medical/dental care, psychiatric services, and legal assistance for disability claims).
